Senior Whole Health NHC (HMO SNP)

Senior Whole Health NHC (HMO SNP) is designed for members who receive home and community-based support services because they have functional deficits. These services help persons who normally qualify for a nursing home (Nursing Home Certifiable) to remain safely at home. 

To be eligible for Senior Whole Health NHC (HMO SNP), members must:

  • Be 65 years old or older
  • Have both Medicare Part A and B, and MassHealth Standard
  • Live in Bristol, Essex, Hampden, Middlesex, Norfolk, Plymouth, Suffolk or Worcester county
  • Select a network Primary Care Physician and agree to assist their Primary Care Physician in developing a plan of care

Medicare Advantage Special Needs Plan (SNP)

Senior Whole Health (HMO SNP) is a Medicare Advantage Special Needs Plan that combines your Medicare benefits and MassHealth Standard benefits, including Medicare Part D, all in one convenient plan. And, at no cost to you.

To be eligible for Senior Whole Health (HMO SNP), members must:

  • Be 65 years old or older
  • Have both Medicare Part A and B, and MassHealth Standard
  • Live in Bristol, Essex, Hampden, Middlesex, Norfolk, Plymouth, Suffolk or Worcester county

Senior Care Options (SCO) Program

For seniors with MassHealth Standard only, Senior Whole Health's Senior Care Options (SCO) program offers the same benefits as Senior Whole Health HMO SNP. And, at no cost to you.

The plan provides you with all your MassHealth Standard benefits, plus more.

To be eligible for Senior Whole Health's Senior Care Options program, members must:

  • Be 65 years or older
  • Have MassHealth Standard
  • Live in Bristol, Essex, Hampden, Middlesex, Norfolk, Plymouth, Suffolk or Worcester county
