Welcome Senior Whole Health (SWH) Senior Care Option members. Thank you for choosing Senior Whole Health as your Senior Care Options (SCO) plan.

Senior Whole Health Personal Care Team

Every member is assigned a Senior Whole Health Personal Care Team that includes a SWH Nurse Care Manager, Primary Care Provider, a SWH Community Resource Coordinator and community service agencies. Our members are also supported by professionals such as SWH pharmacists and social workers. 

Your Senior Whole Health Personal Care Team coordinates every aspect of your care, every step of the way, to meet your individual needs. 

As a member, you receive home assessments to evaluate your needs from our Personal Care Team, including a nurse care manager and community resource coordinator. 

  • Nurse Care Manager — Helps you find your way around the health care system, gets answers to your questions and coordinates home care support services to keep you living at home.
  • Community Resource Coordinator — Helps you coordinate community services such as home care services and adult day programs.
  • Geriatric Support Services Coordinator — A local representative from your elder agency that arranges home care services.

You have 24/7 phone access to a Senior Whole Health Nurse Care Manager.
