We want to know! Senior Whole Health conducts Member Advisory Committee meetings to share information with our members.

The meeting is scheduled for Friday, Aug. 14, 2020 from 11:30 a.m. until 1 p.m. Topics will include health resource tools, medication adherence, and the impact of COVID-19.

Members will have time for an open discussion that will help facilitate SWH improvements.

All members and caregivers are invited to join us virtually using a Zoom meeting on the computer.

Senior Whole Health Personal Care Team

Every member is assigned a Senior Whole Health Personal Care Team that includes your Primary Care Provider, a SWH Nurse Care Manager, a SWH Community Resource Coordinator and local elder agencies. Our members are also supported by professionals such as SWH pharmacists and behavioral health specialists, as well as others important to your care.

  • Nurse Care Manager — Helps you find your way around the healthcare system, gets answers to your questions and coordinates home care support services to keep you living at home.
  • Community Resource Coordinator — Helps you coordinate community services such as home care services and adult day programs.
  • Geriatric Support Services Coordinator (GSSC) — A local representative from your elder agency that arranges home care services.
Your Senior Whole Health Personal Care Team coordinates your care, every step of the way to meet your individual needs. 

As a member, you receive home assessments by your SWH Nurse Care Manager and your GSSC to evaluate your needs.

You have 24/7 phone access to a Senior Whole Health Nurse Care Manager.
