Welcome MLTC Members
The MLTC Plan provides benefits and service to adults 21 or older who are Medicaid eligible and need long term support and assistance with day-to-day activities but choose to remain living at home. Members must live in the SWH of NY service area which includes Bronx, Kings, New York, Queens and Westchester counties.
Membership in Senior Whole Health of New York's MLTC Plan does not change your coverage for important medical benefits such as ambulance, doctor visits, emergency room care, hospitalizations and prescription drug coverage. Members with Medicare will continue to be covered by Medicare for these services. And, members using their Medicaid card when receiving these services will also still be covered.
As a member, you have a dedicated care manager and 24/7 phone access to a Senior Whole Health of New York Nurse Care Manager.
Your Senior Whole Health of New York Nurse Care Manager will work with you to coordinate your benefits and services to ensure you are getting the care you need. It's just one of the ways we provide you with the attention you deserve.
