Welcome Senior Whole Health of New York NHC (HMO SNP) members. Thank you for choosing Senior Whole Health of New York NHC as your Medicare Advantage (HMO SNP) Medicaid Advantage Plus Plan.

The Senior Whole Health of New York NHC Plan provides care to chronically ill seniors 65 or older who have both Medicare Part A and Part B, full Medicaid, require long-term care services for more than 120 days, and who live in our service area. Our service area in New York City includes Bronx, Kings, Nassau, New York, Queens and Westchester counties.

Senior Whole Health of New York's NHC plan offers comprehensive care to keep members living independently at home rather than residing in a nursing home, hospital or other long term care facility.

Your Senior Whole Health of New York Personal Care Team is here to help you receive quality medical, social and community based services. Your team works with you to create an individual care plan.
